- Unlimited office/ telemedicine visits- do not stress if a follow-up is needed you will not have a copay or extra fee

- Annual physical exams- longer comprehensive visit
- Preventative care screening and coordination per USPSTF guidelines (the screening test or procedure is not included and may require a referral to a specialist)
- Chronic disease management - maintenance is key to staying healthy! Comprehensive care plans will help you feel your best and and stay independent and active as possible
- Weight loss management
- Tobacco and alcohol counseling
- Anxiety/ depression management
- Specialty care referrals and coordination where appropriate - it is important to know when specialty care is needed since too many referrals results in too many cooks in the kitchen
